SCHOLARSHIP GUIDELINES
1. All students who provide proof of graduation from
an accredited Indianapolis area high school are eligible to apply for a scholarship.
2. Scholarship recipients are selected by the Scholarship
Awards Committee.
3. All students who wish to be considered for a scholarship
must send an official transcript from their high school. (Transcripts MUST display the school seal and it must be signed by
the proper school official.) Each student must submit a typewritten letter of introduction. The letter must include the student’s
full name, complete address, zip code, telephone number, college/university or vocational school of their choice, and their
major/minor areas of study they plan to pursue. A recent 2”X 3” photograph must be attached. Three letters of
references are required and must include the names, addressees and telephone numbers of the persons providing the references.
4. An applicant must submit proof of acceptance to
an accredited college/university or vocational school.
5. All applications meeting these guidelines will
be reviewed by the Scholarship Awards Committee. Each applicant will participate in a personal interview with the committee.
6. Scholarship recipients will be notified
by the Committee and scholarships will be issued in the name of the recipient and his/her choice of college /university or
vocational school. The scholarships will be presented (date to be announced) after the Cosmo Knights Junior and Scholarship
Golf Tournament, which will be held the 4th Friday & Saturday in July each year @ Douglas Golf Course.

Applicants are encouraged to carefully read the guidelines and complete the attached application form. An incomplete
application will automatically disqualify an applicant. Scholarship applications will be available at the Cosmo Knights Club,
3338 North Illinois Street. All applications and required documentation must be received or post marked by: July 15,
each year.
8. All applications and documentation becomes the property of the Cosmo Knights Scholarship
Fund, Inc.
